RESIDENT INFO SHEET
The Resident Information Sheet is kept in the front of the resident’s record.  Make a second copy that you can hand to Emergency Services personnel in case of a 911 call for the resident.

Care Level Needs Summary
This form is useful when a prospective resident is inquiring about your home.
Use it in conjunction with your Services Summary and Fee Schedule (see below) to negotiate private payment rates.

Sample Private Pay Service Summary and FEE Schedule
This sample fee schedule outlines 1) what products and services the care home provides for the monthly fees charged to the resident, 2) what other products and services the resident may need, and that they will have to pay separately for, and 3) the actual fee schedule for residency in the Care Home. * Remember that YOU are the business owner, and can make adjustments to your fee schedule.  If you wish to charge more to a resident that currently lives in your Care Home, you must do so in accordance with written policies and fee schedules, and give a 30 day notice of fee changes except in emergency situations. Be sure to refer to your specific license rules regarding fee schedule changes.
